We see patients 14 and older, which covers most high school athletes and working teens.
We do physicals for school, sports, work, and pre-employment, along with pre-operative clearances when a surgeon needs one before a procedure. Bring the form your school, employer, or surgical office gave you and we’ll complete it during your visit.
The form that needs to be filled out, a list of any medications you take, and your glasses or contacts if you wear them.
